Building Inspector CERT# AE-15-056-BI-R-07
(CDPHE #20929) Expires 12.16.16
Background:
Helpers Disaster Restoration LLC has bulk sampled for asbestos in proposed areas of
Renovation / Demolition at 189 Capitol Ave in Carbondale, CO.
Date of Inspection: 8.18.16
Date of Report: 8.31.16
Field Procedures and Analysis Methods:
Guidelines used for the inspection were established by the Environmental Protection
Agency (EPA) in the Guidance for Controlling Asbestos Containing Materials in
Buildings, Office of Pesticides and Toxic Substances, DOC #560/5-85-024 and 40 CFR
Part 763, Asbestos Hazard Emergency Response Act (AHERA).
Field information was organized as per the AHERA concept of Homogeneous Area. A
Homogeneous Area is defined as a suspect material of similar age, appearance,
function and texture. Each material was grouped together as a specific Homogeneous
Area, sampled and then assessed for condition.
Bulk samples of suspect ACM were analyzed by Polarized Light Microscopy (PLM).
Scope of work:
Proposed renovations deem sampling for Asbestos necessary, as demolition and
removal of such materials will render them friable. Suspect Asbestos containing

building materials were sampled and inventoried for quantity, condition,
and friability.
Materials examined include:
Drywall / Surfacing Material
Homogeneous Areas:
FA1 – 5 // Drywall
( Surfacing Material / Miscellaneous Material )
Sample Locations:
FA1 // bath 1 at doorway under outlet
FA2 // entry room wall under wolf picture
FA3 // hallway closet at corner
FA4 // master bedroom at electrical outlet by moon picture
FA5 // master bathroom under towel rack
Conclusions:
Laboratory analysis confirms samples FA2 + 3 contain 3 – 4% Chrysotile in the joint
compound of the wall system. This appeared to be homogenous throughout the
unit. There was approximately 510 square feet of this material in the area of
proposed work. The material at the time of inspection was in good condition and
non-friable.
