Construction Management Plan Project Address – 800 Mill Street, Unit #10 – Fifth Avenue Condominiums Project overview – as this is an interior remodel on a condominium building, several items from the standard CMP are not applicable. Per the attached plans, the entire interior of the unit is going to have finishes removed and replaced. This submittal follows the CMP Checklist 1.0 General – not applicable as all work is interior 2.0 Project Introduction – not applicable as there is no disturbance beyond the interior work. Vicinity map attached Exhibit “A” 3.0 Project Documentation – see attached plan submittals, complete with existing and proposed conditions, HOA approvals, and required documents. 3.1 Permit Numbers TBD upon issuance 3.2 Project Notification – neighboring units will be notified by the HOA, and CCM shall contact direct neighbors as work is ongoing 3.3 Project Sign – see attached Exhibit “B” 3.4 Contact List – see Building Permit Application for all contacts 4.0 Project Implementation – 4.1 Dates of Construction – 5 Months, start date depends on Permit 4.2 Hours of Construction - Construction hours will be limited to 7:30am – 5:30pm Monday through Friday and 9:00am- 5:00pm on Saturday. All activities generating noise in excess of 80 decibels will require a noise suppression plan and be restricted to operating between 9:00am – 5:00pm Monday through Friday. Additional prohibited construction days are as follows: • Sundays • Food and Wine Weekend (Friday-Sunday) • 4th of July (Day and any associated weekend) • Memorial Day Weekend • Labor Day Weekend • Thanksgiving Day • Christmas Week (12/25 – 1/1) 4/16/2018 4.3 Construction Schedule – See attached Schedule Exhibit “C” 5 Months, start date depends on Permit 4.5-4.7 – Not applicable as related to Parks, Trees, and Fencing 5.0 Site Management 5.4 Map – HOA shall designate areas available for staging, parking, and toilet. Our plan is to remove finishes into a trailer and haul off immediately, and all materials delivered to be stored in the unit for security. 6.0 Traffic Control – not applicable as no large trucks are required for this project. Deliveries shall be in small trucks or pickup trucks 8.0 Sediment and Erosion Control – not applicable for this work 9.0 Fugitive Dust Control – Dust shall be contained to the unit and we will use vacuums or containment to control dust. We also use sweeping compound on the floor to help control dust. Our 3rd party safety inspector provides plans and equipment guidance for this 10.0 Emissions – not applicable for this work 11.0 Noise Suppression – General The noise limit for construction is 80 decibels measured at the property line of the construction site. All construction equipment shall be adequately muffled and maintained to minimize project noise. The installation of noise barriers is the minimum technique to suppress noise, especially when jack hammering of concrete occurs. 11.1 - Noise Suppression Plan All activity that generates noise in excess of 80 decibels requires a noise suppression plan and is restricted to operating between the hours of 9am and 5pm Monday through Friday. Neighboring occupants will be notified of noise prior to work being done as a courtesy. Contractor shall use blankets as necessary to contain noise and unit door shall be closed when not in use Please feel free to contact me with any addenda to this plan.
