As a service and equipment provider, we strive to be competitive, but most important of all, to provide our clients with superior service. General. Scope of Work: CD will provide all Labor and Material to stage, erect and dismantle all scaffolds to meet or exceed O.S.H.A. Guidelines. We will provide a safe platform to allow the customer to perform the intended scope of work. This proposal represents a bid to be performed on a Lump Sum basis. CD will provide supervision, material, labor, freight and consumables to stage, ered and dismantle systems scaffolding. CD is to Stage, Erect andD' mantle the some listed below 1.) CD Specialty Contactors will provide a 3' diameter poly trash chute 30' H with intake at the top. 2.) Prior installation of poly trash chute, CD Specialty Contractors will take a mandatory Safety meeting to discuss tie -off points. 3.) Trash chute will be assembled at ground level and installed using winch support. Trash chute wlll be hung on the scaffold rails. Stair tower dimensions are 12W x 14L x 30H. 4.) Pricing includes a plywood wrap at the base of the scaffold to keep un-aumonze personnel from climbing or walking on the scaffold. 5.) CD shall have dear access to erect scaffold, any shifting, alterations or changes to scope required by American Heritage Restoration will be considered extra and will incur additional charges 6.) Scaffold shall include deck with guardrails, We boards and access ladder until intended height is reached to meet and or exceed O.S. H.A. Standards. 7.) American Heritage Restoration is to get any permits that are required to build scaffold on properties. 8.) Pricing does not include site safety orientation nor background checks. 9.) Upon completion, customer must be present to inspect scaffold and sign turd over sheet. Any modifications or alterations after C&D left the job site will be consider change orders. 10.) This proposal is based upon a one-time erection and dismantling of scaffolding per the indicated sections above. Any shifting, alterations or changes to the agreed scope required by American Heritage Restoration will be considered extra and will incur additional charges. 2. Scope and pricing are based on continuous work. Delays. interruptions or modifications to the work scope will be considered extra and will incur additional charges. 3. The scaffolds will be erected for Ight duty use.4fig will be rated -for 25,flsf. It is the responsibility of American Heritage Restoration to ensure that the scaffold limitations are not exceeded. If the customer requests a medium or heavy duty rated scaffold, prior notification must be given to CO. Additional charges may apply. 4. All scaffolds will meet or exceed O.S.H.A. 1926,451 standards. 5. Any scaffold that is moved altered erected or dismantled by any person other than those under the direct supervision of"CD" shall negate the indemnity that is extended in this proposal. 6. Standard Scaffold Consumables — Includes nails, #9 wire, 2x4 and plywood used for O.S.H.A. scaffold platform construction only. Any additional 2x4 or plywood requested for purposes other than scaffold platform use will be billed at cost 00%with prior approval. Materials such as tales, visqueen, or weather enclosure wrap will be billed at cost 10%with prior approval. 7. CD shall provide all freight in and out for this project. 8. prior to removal of any scaffolding, a final cleaning of the scaffold is to be conducted by American i eritsae Restoration. 9. American Her Was Restoration will be responsible that no pedestrians or un-authorized personnel will be on the scaffold. 10. It will tee the responsibility of American Heritage Restoration to supply a place to efficiently unload the material truck that is within 100 Ft. of the build site. Disruptions to this specification may incur adtlitional charges. 11. American Heritage Restoration is to provide one parking space for a personal vehicle for CO Specialty Contractor's crew on the days of the scaffold erection and scaffold dismantle. 12. American Heritage Restoration will be held responsible for keeping a man on site in the moming to show the scaffolding crew where the build site is located. This man will need to re -visit the crew upon completion to accept the scaffold will suffice his needs. Extra mobilizations may incuradditional costs. 13. American Heritage Restoration shall sign a scaffold turn over sheet provided by CD Specialty Contractors once the scaffold is erected and meets the customer's needs, any changes may incur additional costs. American Heritaas Restoration will be responsibe for scaffold inspections once hand over sheet has been signed and accepted Schedule: 1.
