CLIENT BACKGROUND
The Client, 800 S Mill 303, LLC., hired HealthSafe Inspections, Inc. (hereinafter,
HealthSafe) to perform a post-abatement clearance for asbestos containing sheet vinyl
flooring which was abated, that is, removed entirely from the kitchen and adjacent
bathroom. This required an asbestos abatement permit from CDPHE (Permit Notice
#18P12525A dated 04/30/2018) and a certified asbestos abatement. Prestige
Environmental Services, Inc. provided the asbestos abatement and called for the
abatement clearance inspection and PCM air sampling once their abatement of the sheet
vinyl flooring layers was completely removed and disposed of.
2
VISUAL INSPECTION & LABORATORY FINDINGS
An asbestos post-abatement visual clearance inspection and PCM air sampling was
conducted by Jim Baker of HealthSafe on Friday May 18, 2018. The visual clearance
verified that all friable sheet vinyl flooring had been completely removed from the
plywood subflooring in the kitchen and adjacent bathroom. All debris was removed and
the remaining subfloor was virtually dust-free. No further abatement, removal or cleaning
procedures were required.
Following the visual clearance, clean rotary vane sampling pumps were warmed up for 5-
10 minutes and site calibrated. A box fan was deployed at a 45°angle off the east wall of
the bathroom and the containment area blown with a leaf blower as required by the
regulations for clearances. PCM samples were set in locations to represent the
distribution of air throughout the containment-abatement space. A trip blank and two field
blanks, opened for 30 seconds, were included in the batch of five PCM air samples, but
were not required to be analyzed by the laboratory.
3
HEALTHSAFE PCM LABORATORY DATA
The five PCM air samples at total volumes between 930 and 941 liters of air all had less
than 0.005 fibers detected (<0.005 f/cc) per cubic centimeter of air. This well below the
0.01 f/cc level for clearance.
See attached Hayes laboratory report #18015520.
CONCLUSIONS
Based on the visual post-abatement verification clearance inspection and based on the
post-abatement PCM air sample data, the containment area of the dining and bathrooms
spaces has fully passed abatement clearance.
The containments and equipment can be disassembled and removed. Repairs and/or
remodeling can commence at once.
Sincerely Submitted,
Jim Baker

All samples were received in acceptable condition unless otherwise noted on the report. The Report must not be used by the client to claim product certification, approval, or
endorsement by: AIHA, NIST, NVLAP NY ELAP, or any agency. The results relate only to the items tested. The results include an inherent uncertainty of measurement associated with
estimating percentages by polarized light microscopy. Measurement uncertainty data can be provided when requested. None detected: Below the detected reporting limit of 1% unless
point counting is performed, then the detected, reporting limit is .25%. Per NY ELAP198.6 (NOB) TEM is the only reliable method to declare an NOB material as Non-Asbestos
Containing. Hayes Microbial Consulting reserves the right to dispose of all samples after a period of 60 days in compliance with state and federal guidelines.
