No disturbance will occur within the tree protection zones and within the setbacks of the ditch. 2.2 Project Vicinity Map See Attached Appendix A—Project Vicinity Map 2.3 Project Description Scrape existing single family residence(SFR)and replace with a new SFR of approximately 5500 SF. 3.0 PROJECT DOCUMENTATION 3.1 Permitting This Construction Management Plan (CMP) is being submitted as required by the Permitting Process for the Building Permit. 3.2 Public Notification A minimum of 10 days prior to the start of construction,a notification letter will be mailed to all neighboring properties.This letter will provide information about the project,the proposed project and construction schedule,any potential impact to the neighborhood, and finally,an invitation to the Pre-Construction Meeting. Demolition is tentatively scheduled to begin on October 2, 2017. As such, a Pre-Construction meeting will be scheduled for September 25, 2017, 5 working days prior to the start of Construction. This meeting will summarize this CMP and give attendees additional information regarding the project. 3.3 Project Sign A sign sized 36"x24" will be posted at the site. It is estimated that the total construction duration will be 16 Months,with a targeted completion date of March 1, 2019. 4.2 Hours of Construction As required by City Ordinance,construction hours will be limited to 8:00am—7:00pm, Monday throu:• _ and 9:00am — 5:00pm on Saturdays. Construction activities will not be permitted on Sundays. Please adjust to ' current standards constrained to the hours of 9:00am—5:00pm Monday through Friday only. The following days are denoted as No-Work days; Sundays Food and Wine weekend (Friday-Saturday) 4th of July and Weekend Memorial Day Weekend, including Monday Labor Day Weekend Thanksgiving Day Week of Christmas (Dec. 25-Jan. 1) 4.3 Schedule See attached Appendix B for Construction Schedule. 4.4 Adjoining Properties Soil stabilization measures will be taking utilization shoring methods on three sides. 4.5 Project Fencing The Project will be secured with removable and semi-permanent chain link style construction fence for the duration of the Demolition.The fence will be 6'-0"in height with windscreen,and will be installed per the attached site plan. 4.6 Public Health and Welfare Public health and welfare is central to the daily operations of the demolition process. 4.7 Natural Environment Stephen Ellsperman and DHM Design will act as the consulting arborist for the Project and will submit for,and obtained a Tree Removal Permit from the City Forester. 5.0 PARKING MANAGEMENT 5.1 Parking Management See attached Appendix C-Parking Management Plan 5.2 Emergency Access/Ordinance 35 Summerhill Partners will maintain continuous access for Emergency Vehicles (Police, Fire, and Ambulance) on and around the Project site. Reference Appendix G. 5.3 Construction Parking Parking is intended to be within the project site fence. Street parking will also be necessary,and Summerhill will seek those permits as required. Received 5/15/17 5.4 Staging Areas All materials, materials handling equipment,and staging will be confined to the Project site. Summerhill Partners will maintain trash containers and portable restroom facilities within the limits of the property.See attached Site Plan,Appendix D. 5.5 Material Storage and Waste Management Metal and Concrete materials from the demolition will be separated and organized on the site for recycling. 6.0 TRAFFIC CONTROL 6.2 Haul Routes See Appendix A- Haul Route Map 6.3 Weight and Size Limitations of Vehicles The following represents the weight and size limitations for vehicles for the Project; Dump Trucks—54,000 GVW Excavator— 54,000 GVW 6.4 Delivery Requirements We estimate a maximum of two deliveries at a time. Delivery hours would be between 8:00am and 5:00 PM M-F The anticipated numbers of total truck trips is as follows; 6 Loads of Excavation Equipment Mobilization 30 Loads of Construction Debris Leaving Site 75 Temporary Toilet Cleaning Trips Debris pickup and delivery times will be coordinated such that only one truck will be on-site at any given time. Delivery vehicles and all other on-site vehicles will not be allowed to idle for more than 5 minutes, (with the exception of generators or PTO type operations). Project sub-contractor and suppliers will be provided with written notification of Aspen's vehicle idling regulations. See Appendix H -Vehicle Idling Regulations 6.5 Traffic Control Plan Reference Appendix E for TCP. 8.0 SEDIMENT AND EROSION CONTROL 8.1 Storm Water Pollution Prevention Plan All necessary Best Management Practices(BMP's)will be installed and maintained in order to minimize potential contaminants entering adjacent streets,gutters,and waterways. Primary measures are as follows: • Avoid unnecessary site disturbance. • Maintain grading; install silt fences and sediment traps, prior to and during any soil disturbance. • Minimize tracking of dirt and mud outside the project limits through engineering controls and use of tracking pads. • Utilize approved means and methods for concrete placement and washout. Received 5/15/17 • Use approved temporary sanitary facilities, and adhere to maintenance means, methods and schedules. • Stabilize pathways around the perimeter of the project primarily through the use of screened rock over geo- textile fabric. 8.2 BMPs for Storm Water Pollution Prevention Best Management Practices(BMPs)will be employed for storm water management as indicated in the following sections. A. Structural Practices: a. Silt fences will be installed on the downhill sides of all areas of disturbance. (See Fig. 1.0, Typical Temporary Sediment Control Fence.) b. Utilize a Vehicle Tracking Control pad (VTC). c. Temporary diversion ditches and swales will direct runoff from other site areas through straw wattle and hay bale filtration prior to discharge off the site. The exact location and alignment of these structures will be determined in the field based on project phasing and the space needed for construction staging. d. Fugitive dust control will be achieved using a hose and spray nozzle. e. Surface roughening will be conducted in newly excavated areas to provide temporary stabilization when other temporary stabilization measures are not feasible(such as gravel or temporary vegetation)or will not be provided immediately. Non-Structural Practices: Pollution prevention will be stressed among all employees and sub-contractors, including adequate training of personnel and frequent monitoring of activities. a. Proper facilities and procedures will be used for on-site waste storage and disposal. Daily inspection and maintenance of waste disposal areas will be conducted.Where possible,waste containers will be covered, and waste storage areas will be fenced. b. The contractor will ensure that good housekeeping be employed during demolition,with all work, materials storage and waste storage areas kept in a neat and orderly condition. c. Existing trees and mature vegetation that are not to be removed will be protected during demolition to limit unnecessary disturbance of site soils and maintain natural erosion control. D. Temporary Facilities: A temporary sanitary facility(Port-a-potty)will be utilized on-site during construction activities.The following BMP's will be utilized to minimize potential for negative environmental impact; 1. Portable toilets should be located away from high-traffic vehicular areas. 2. Portable toilets must be placed at least 20 feet away from all storm drains and streets. 3. Portable Toilets MUST NOT EVER be placed on top of storm drain inlets or on a street. 4. Portable toilets shall be placed on a level ground surface that provides unobstructed access to users and servicing pump trucks. 5. Portable toilets should wherever possible be located upon natural ground and not on or within 5 feet of a paved surface such as asphalt, concrete or similar. 6. If portable toilets must be placed on a paved surface exposed to rainwater or storm water runoff,extra care must be taken during servicing to ensure any waste water spilled onto the paved surface is rinsed and adequately collected so as not to leave any residue. A wet shop vacuum or similar would provide for adequate collection. 7. Portable toilets should not be located within the buffer of any stream (ditch)or lake,or within any other larger stream/lake buffer that may have been established. The following maintenance steps will be utilized to minimize potential for negative environmental impact; 1. Portable toilets must be cleaned and have waste removed at least weekly.Additional servicing and/or portable toilets may be necessary depending on the volume of use the toilets receive. 2. Damaged portable toilets must be repaired/replaced immediately. Received 5/15/17 3. Rinsing of portable toilets(excluding the inside of portable toilet waste tank) may be completed on site when the following conditions are met: a. Rinse water is controlled to prevent it from entering into storm drain; b. No more than one(1)gallon of rinse water is used per portable toilet(i.e. low volume high- pressure cleaners, or bucket and rag. No common household hoses.); c. rinsing is completed at least 20 feet away from a street or storm drain. d. Where the portable toilet must be located on a paved surface: i. any rinse water that comes in contact with the paved surface must be adequately collected. e. Where the portable toilet is located on a non-paved surface: i. rinsing should be completed at least 5 feet away from a paved surface; ii. rinsing wastewater is drained to the ground at a rate that allows it to immediately soak into the ground; f. Rinse water generated during the cleaning of portable toilet waste tanks must not be discharged to the ground or to a storm drain and must be retained within the tank. g. Maintain all portable toilets in good condition to prevent leaks or spills. h. Portable toilet wastewater(human waste/sewage) must never be disposed of on-site. E. Materials Handling a. All potentially hazardous materials will be stored off site. In the event that any such materials are stored on site,toxic materials and petroleum products will be enclosed and contained in an impervious elevated basin sized large enough to hold the volume of the hazardous materials being contained. b. Any spill or discharge or construction related materials will be cleaned up immediately and disposed of accordingly. c. Any spill not properly contained and cleaned-up will be reported to the proper authorities. i. Aspen Fire Department, Fire Marshal: 970-925-2690. ii. Colorado Department of Public Health and Environment, Emergency Response:303-756- 4455. iii. City of Aspen Engineering Department:970-920-5080. iv. City of Aspen Environmental Health Department:970-920-5039. v. National Response Center:800-424-8802. 8.3 Final Stabilization and Long-Term Storm Water Quality All temporary BMPs will remain in place until demolition is complete and revegetation has been verified to have taken hold. Installation of permanent site drainage measures and storm water management practices will be as detailed in the project Civil Drawings, included in the permit submittal package. 8.4 Inspection and Maintenance Procedures A. A thorough inspection of the storm water management system will be performed at least every 14 days,and after any precipitation event that causes run off.The inspection will include the observation of the construction site perimeter and discharge points,all disturbed areas, and material storage areas exposed to precipitation. All erosion and sediment control measures will be inspected for integrity and functionality(e.g.,sediment fences, hay bales, straw wattles, temporary swales and ditches). B. Damaged or ineffective BMPs will be fixed and/or replaced. Removed sediment should be placed where it will not become an additional pollution source. C. When necessary, BMPs will be added, removed or replaced with more effective measures. All such actions will be documented and the SWMP will be updated to reflect those changes. 9.0 FUGITVE DUST CONTROL 9.1 Dust Control Plan The project's Dust Prevention and Control Plan are as follows: Received 5/15/17 • Avoid unnecessary site disturbance. • During demolition operations if dry or deemed otherwise necessary a hose and spray nozzle will be used on site to mitigate air born dust. • Exported demolition debris will be hauled off via covered dump trucks.All areas directly surrounding the site will be maintained daily with thorough cleanings taking place every Friday afternoon. • 24 Hour Contact: Rob Cairncross 970-618-6722 10.0 EMISSIONS 10.1 Maintenance All vehicles and equipment used on site will be properly maintained such that the engines will function within the manufacture's standards or parameters. 10.2 Opacity Emissions from diesel engines operated within the City of Aspen shall be of a shade or density no darker than 40% opacity,except for starting motion no longer than 10 seconds or for stationary operation not exceeding 10 seconds. 11.0 NOISE SUPPRESSION 11.1 General The noise limit for construction is 80 decibels measured at the property line of the construction site. All construction equipment shall be adequately muffled and maintained to minimize project noise. The installation of noise barriers is the minimum technique to suppress noise, especially when jack hammering of concrete occurs 11.2 Noise Suppression Plan All activity that generates noise in excess of 80 decibels requires a noise suppression plan and is restricted to operating between the hours of 9am and 5pm Monday through Friday. See Noise Suppression Plan-APPENDIX F. 11.3 Equipment and Methods In addition to the standard practices agreed to within the signed Noise Suppression Plan,the table below from the City of Aspen's complete packet has been included for quick contractor/subcontractor reference. These plans are required for all residential construction or remodeling'of 1,500 square feet,or more, for any construction or remodeling involving multi-family buildings (three Or more units), or for any construction or remodeling involving commercial buildings. All other construction activity shall be considered exempt from these provisions. The construction project shall be issued street parking permits for the expected duration of the project for a fee. Parking permits issued, Pursuant to an approved Construction Staging Area, Parking, and Emergency Vehicle Access Management Plaa3, shall permit the all-day on-street parking of construction related vehicles within the specified construction parking area for the duration of the construction project. Construction vehicles parked outside of the approved area or after the specified construction duration shall,be considered in violation. The Chief Building Official shall require submission of a Construction Staging Area, Parking, and Emergency Vehicle Access Management Plan with submission of building permit documents. (a) Except as hereinafter provided, it shall be unlawful for any person to idle or permit the idling of the motor of any stationary motor vehicle for a prolonged or unreasonable period of time determined herein to be five (5) minutes or more within any one(1) hour period of time. (b) This section shall not apply when an engine must be operated in the idle mode for safety reasons including, but not limited to, the operation of cranes and fork lifts used in the construction industry. (c) The time required by a diesel powered motor vehicle with a gross weight rating of ten thousand (10,000) pounds or more while operating in a stationary position to achieve a temperature of one hundred twenty (120) degrees Fahrenheit and an air pressure of one hundred (100) pounds per square inch, shall not be included in the computation of the five(5) minutes determined herein to be a prolonged or unreasonable period of time. The temperature and air pressure as indicated on the vehicle's gauges may be used for determining the diesel engine's temperature and air pressure. (d) The time during which transportation vehicles are actively loading or discharging passengers shall not be included in the computation of the five (5) minutes determined herein to be a prolonged or unreasonable period of time. A transportation vehicle shall be defined for purposes of this section to mean motor vehicles designed to transport a minimum of sixteen (16) persons. (Ord.
