The second phase will include placement of micro piles,construction of a new foundation and the new residence. No major disruption or negative effect to public utilities is anticipated. The demolition process will require temporary abandonment of the sewer line until new sewer service is established. In addition, during the water main tap process,water service may need to be briefly disrupted for downstream customers for a few minutes to connect the new water supply. 3.0 PROJECT DOCUMENTATION 3.1 Permits/Other Documents This Construction Management Plan (CMP) is being submitted as required by the Permitting Process for the Building Permit. Koru Ltd. operates as a General Contractor in the City of Aspen and Pitkin County under License No. C-005616 See attached Appendix H —Contractor's License. When available, the Building Permit will be posted on site in addition to any ancillary permits for ROW easements, etc. PUD, Subdivision Improvement Agreements and Related City Ordinances are incorporated into the plan set, as required. 3.2 Public Notification A minimum of 10 days prior to the start of construction, a notification letter will be delivered to all neighboring properties with in a 300 foot radius. This letter will provide information about the project, the proposed project and construction schedule, potential impact to the neighborhood and an invitation to the Pre-Construction Meeting. In addition, required personnel including Utility Personnel, applicable City Departments, the Roaring Fork Transit Authority and the Aspen School District will be notified and invited to attend the Pre-Construction Meeting. Demolition will begin upon receipt of the building permit which is anticipated on or around July 10, 2017. As such, the Pre-Construction Meeting will be scheduled for July 1, 2017. This public forum meeting will summarize this CMP and give attendees additional information regarding the Project. During the construction process, public updates will be available at a kiosk or information box located near the front gate or office. In addition, should major events where the public may be affected (e.g. road closures, parking leases, ROW usage, etc.) signage will be placed on site and made visible to the public and direct notifications made to those community members who are available and have the potential to be directly affected by construction activities (immediate neighbors). It is estimated that the total construction duration will be 24 months,with an anticipated completion date on or about July 17, 2020. 4.2 Hours of Construction As required by City Ordinance, construction hours will be limited to 7:30am —5:30pm, Monday through Friday, and 9:00am —5:00pm on Saturdays. Construction activities will not be permitted on Sundays. In accordance with the Noise Suppression Plan, work activities generating noise levels in excess of 80 db. will be constrained to the hours of 9:00am —5:00pm Monday through Friday only. The fence will be 6'-0" in height with windscreen and will be installed per the attached site plan. In addition, where and when applicable, noise suppression fencing will be utilized to delineate project boundaries and adhere to noise mitigation requirements. 4.6 Public Health and Welfare Public health and welfare are of paramount importance within the active management and daily operations of the construction process. The management practices contained within this CMP detail the steps that will be taken to maintain public safety and minimize any impact the Project may have. 4.7 Natural Environment A tree mitigation plan will be prepared by the Architect and submitted with the Plans. This mitigation plan will detail proposed tree removals and trees to be protected in place. The City Forester will be directly involved in the approval and verification process for this effort. 5.0 SITE AND RIGHT OF WAY MANAGEMENT 5.1 Right of Way Management Plan See attached Appendix G— Proposed Site Management Plan. We do not anticipate use of the ROW for construction related activities. 5.2 Emergency Access KORU, Ltd. will maintain continuous access for Emergency Vehicles (Police, Fire, and Ambulance) to the Project site. See attached Appendix G— Proposed Site Management Plan, for diagram detailing Emergency Ingress/Egress for the site. 5.3 Construction Parking Details See attached Appendix G— Proposed Site Management Plan. The onsite parking and trailer location are inside of the property line. Sub-contractors will be required to carpool, minimizing the impact on city parking. There are a few parking places on-site as noted on the CMP. See attached Appendix G. Koru encourages its subcontractors to take advantage of the City Carpool program in an effort to minimize the number of vehicles present on a daily basis. 5.4 Staging Areas All materials, handling equipment and staging will be confined to the Project site. Under no circumstances will Project materials or equipment be stored in the Public ROW. KORU, Ltd. will maintain trash containers and portable restroom facilities within the limits of the Project. See Appendix G— Proposed Site Management Plan 5.5 Right of Way Limitations Any pedestrian ROW interactions at the Black Birch Drive and Red Butte Drive property line(s) will be handled with pedestrian safety as the primary concern. No food refuse will be stored on the site. A waste management plan will be provided to the City prior to demolition. 6.0 TRAFFIC CONTROL 6.1 General All traffic control operations will be managed by the designated Traffic Control Supervisor(see contact designation) 6.2 Haul Routes See Appendix B - Project Vicinity and Haul Route Map 6.3 Onsite Vehicle Limitations The following represents the weight and size limitations for vehicles for the Project; Dump Trucks—54,000 GVW Excavator—54,000 GVW Boom Truck—45,000 GVW 6.4 Delivery Requirements We estimate a maximum of two deliveries at a time. Delivery vehicles and all other on-site vehicles will not be allowed to idle for more than 5 minutes, (with the exception of generators or PTO type operations). Project sub-contractor and suppliers will be provided with written notification of Aspen's vehicle idling regulations. If an unforeseen need arises, a Traffic Control Plan (TCP will be prepared by a State Certified Traffic Control Supervisor. This plan will conform to the most current edition of the Manual of Uniform Traffic Control Devices for Streets and Highways (MUTCD) and be submitted to the City of Aspen Department of Engineering for approval/permitting. 7.0 PEDESTRIAN PROTECTION 7.1 General Pedestrian protection will be a major concern throughout the project. 7.2 Limitations No limitations are anticipated in consideration of the proposed project limits. 8.0 SEDIMENT AND EROSION CONTROL 8.1 Requirements The Project Civil Engineer has provided an engineered storm water management system that complies with the City's Urban Runoff Management Plan. All other Sediment and Erosion Control related activities will be managed using the following measures: Storm Water Pollution Prevention Plan All necessary Best Management Practices (BMP's) will be installed and maintained in order to minimize potential contaminants entering adjacent streets, gutters, and waterways. Primary measures are as follows: • Avoid unnecessary site disturbance. • Maintain grading; install silt fences and sediment traps, prior to and during any soil disturbance. • Minimize tracking of dirt and mud outside the project limits through engineering controls and use of tracking pads. • Utilize approved means and methods for concrete placement and washout. • Koru will make every effort to take additional precautionary actions by limiting site ingress/egress during and immediately following significant precipitation events, and by timing egress (during winter months) to morning hours prior to daily thaw cycles. • Use approved temporary sanitary facilities, and adhere to maintenance means, methods and schedules. BMP's for Storm Water Pollution Prevention Best Management Practices (BMPs)will be employed for storm water management as indicated in the following sections. A. Structural Practices: 1. Silt fences will be installed on the downhill sides of all areas of disturbance. 2. Koru Ltd will install a Vehicle Tracking Control pad (VTC). i. Koru will make every effort to take additional precautionary actions by limiting site ingress/egress during significant precipitation events, and by timing egress (during winter months) to morning 3. Temporary diversion ditches and swales will direct runoff from other site areas through straw wattle and hay bale filtration prior to discharge off the site. The exact location and alignment of these structures will be determined in the field based on project phasing and the space needed for construction staging. 4. Fugitive dust control will be achieved using a hose and spray nozzle. Surface roughening will be conducted in newly excavated areas to provide temporary stabilization when other temporary stabilization measures are not feasible (such as gravel or temporary vegetation) or will not be provided immediately. B. Inlet/ Drain / Stockpile Protection Following installation of any temporary and / or permanent storm water drains, sump pits, or swales, temporary sediment control features will be installed and maintained until project completion. Non-Structural Practices: Pollution prevention will be stressed among all employees and sub-contractors, including adequate training of personnel and frequent monitoring of activities. 1. Proper facilities and procedures will be used for on-site waste storage and disposal. Daily inspection and maintenance of waste disposal areas will be conducted. Where possible,waste containers will be covered, and waste storage areas will be fenced. 2. The contractor will ensure that good housekeeping be employed during demolition. All work, materials storage and waste storage areas will be kept in a neat and orderly condition. 3. Existing trees and mature vegetation that are not to be removed will be protected during demolition to limit unnecessary disturbance of site soils and maintain natural erosion control. D. Temporary Facilities: A temporary sanitary facility(Port-a-potty) will be utilized on-site during construction activities. The following BMP's will be utilized to minimize potential for negative environmental impact; 1. Portable toilets should be located away from high-traffic vehicular areas. 2. Portable toilets must be placed at least 20 feet away from all storm drains and streets. 3. Portable Toilets MUST NEVER be placed on top of storm drain inlets or on a street. 4. Portable toilets shall be placed on a level ground surface that provides unobstructed access to users and servicing pump trucks. 5. Portable toilets should wherever possible be located upon natural ground and not on or within 5 feet of a paved surface such as asphalt, concrete or similar. 6. If portable toilets must be placed on a paved surface exposed to rainwater or stormwater runoff, extra care must be taken during servicing to ensure any waste water spilled onto the paved surface is rinsed and adequately collected so as not to leave any residue. A wet shop vacuum or similar would provide for adequate collection. Portable toilets should not be located within the buffer of any stream, ditch, or lake, or within any other larger stream/lake buffer that may have been established. 8. Portable toilets must be cleaned and have waste removed at least weekly. Additional service intervals and/or additional portable toilet units may be necessary depending on the volume of use the facilities receive. 9. Damaged portable toilets must be repaired or replaced immediately. 10. Rinsing of portable toilets (excluding the inside of portable toilet waste tank) may be completed on site when the following conditions are met: 11. Rinse water is controlled to prevent it from entering into a storm drain; E. Handling 1. All potentially hazardous materials will be stored off site. In the event that any such materials are stored on site, toxic materials and petroleum products will be enclosed and contained in an impervious elevated basin sized large enough to hold the volume of the hazardous materials being contained. 2. Any spill or discharge or construction related materials will be cleaned up immediately and disposed of accordingly. 3. Any spill not properly contained and cleaned-up will be reported to the proper authorities. a. Aspen Fire Department, Fire Marshal: (970) 925-2690. b. Colorado Department of Public Health and Environment, Emergency Response: (303) 756-4455. ii. City of Aspen Engineering Department: (970) 920-5080. iii. Final Stabilization and Long-Term Storm Water Quality All temporary BMPs will remain in place until construction is complete, and revegetation has been verified to have taken hold. Installation of permanent site drainage measures and storm water management practices will be as detailed in the project Civil Drawings, included in the permit submittal package. Inspection and Maintenance Procedures A. A thorough inspection of the storm water management system will be performed at least every 14 days, and after any precipitation event that causes run off. The inspection will include the observation of the construction site perimeter and discharge points, all disturbed areas, and material storage areas exposed to precipitation. All erosion and sediment control measures will be inspected for integrity and functionality (e.g., sediment fences, hay bales, straw wattles, temporary swales and ditches). B. Damaged or ineffective BMPs will be fixed and/or replaced. Removed sediment should be placed where it will not become an additional pollution source. C. When necessary, BMPs will be added, removed or replaced with more effective measures. All such actions will be documented and the SWMP will be updated to reflect those changes. 8.2 Smuggler Mountain Restrictions Not applicable for this project 9.0 FUGITVE DUST CONTROL 9.1 Dust Control Requirements The project's Dust Prevention and Control Plan are as follows: • Avoid unnecessary site disturbance. • During demolition operations if conditions are dry or deemed otherwise necessary,a hose and spray nozzle will be used on site to mitigate potential airborne dust. • Exported demolition debris will be hauled off via covered dump trucks. All construction equipment est of shall be adequately muffled and maintained to minimize project noise. The installation of noise barriers is the r. minimum technique to suppress noise, especially when shoring and performing other specific localized area tasks. 11.2 Noise Suppression Plan All activity that generates noise in excess of 80 decibels (65 decibels during the summer season) requires a noise suppression plan and is restricted to operating between the hours of 9am and 5pm Monday through Friday. Koru will install noise reduction panels to the perimeter site fence (Behrens and Associates STC-25 Acoustical Blankets or approved alternate) and utilize portable sound blocking panels (Echo Barrier, acoustic noise barrier, or similar) to further mitigate sound as needed at specific localized areas, e.g.: surrounding compressors, mixers, shoring drill. The noise reduction panels will remain on the perimeter site fence from project initiation through the completion of foundation. Throughout the course of the project, the portable sound blocking panels will be placed in the following locations: See Appendix C - Noise Suppression Plan. • Shoring — in front of/ above the drilling rig, alongside the auger at its point of entry into the earth, and along ide the drilling rig. • Mixer—on all sides other than one slot for access to and from the mixer. The opening will be oriented towards the center of the site. • Concrete Saws and Coring —on all sides during cutting/coring. • Compressors—on all sides when outside of the structure. • Table Saws/ Cutting Areas—on all sides when outside the structure other than one slot for access to and from the cutting area. The opening will be oriented towards the center of the site. 11.3 Continued Noise Monitoring In addition to the standard practices agreed to within the signed Noise Suppression Plan, the table below from the City of Aspen's complete packet has been included for quick contractor/subcontractor reference in Appendix C— Noise Suppression Plan. 11.4 Requirements for Activities Exceeding 80 Decibels Activities exceeding 80 Decibels will be scheduled, communicated to neighbors, executed and minimized according to City requirements. Large scale works such as excavation will utilize a temporary noise barrier (Behrens and Associates STC-25 Acoustical Blankets with integral panels or approved alternate) located at the perimeter of the site. Smaller, more localized activities producing excessive noise such as compressors, and mixers,will utilize portable sound blocking panels (Echo Barrier, acoustic noise barrier, or similar). These barriers will be placed to protect the neighboring properties. Notify neighbors within two hundred fifty(300)feet of the project informing them of the kinds of equipment,expected noise levels and durations of loud work. Including the variation of noise levels during a typical construction days may be helpful.Such notification must be in writing and be done seven(7)days prior to the starting time of the project. Communication with neighbors can prevent complaints from arising,and resolve concerns before there is a problem.Provide a phone number where the foreman can be reached prior to the start of the job. 2. Operate equipment in accordance with manufacturer's specifications and with all standard manufacturers'mufflers and noise-reducing equipment in use and in properly operating condition. 3. Post notices to inform workers,including sub-contractors,about the basic noise requirements,as well as specific noise restrictions,to the project. 4. Install noise barriers around all equipment/activities specified in Table 1:TYPICAL CONSTRUCTION EQUIPMENT REQUIRING NOISE SUPPRESSION.Noise barriers not only significantly reduce construction noise,but they also provide an extra benefit of"hiding"the noise producing sources,thus increasing a neighbor's tolerance.(See also the attached list of noise blocking methods for details on approved noise barriers.) 5. Move portable loud equipment including generators,compressors,and cement mixers to different sides of the property to reduce impacts on individual neighbors. What can you do about construction noise? • The use of radios on the site before 8:00 am are not allowed:remind laborers and sub- contractors. • Noise suppression plans will not allow noisy machines such as brick cutters or jackhammers to be operated before 9.00 am. • Noisy equipment such as cement mixers should be placed on the site to maximize the distance from neighboring houses and/or rotate location so as to not impact just one neighbor.Noise levels drop quickly with distance from the source. • All equipment should be properly maintained,with special attention to mufflers and other noise control devices. • Between work periods,builders are required by city ordinance to shut down machines such as backhoes,bobcats, loaders and generators. • When dropping materials from a height—for example,into or out of a truck,or when loading or unloading scaffolding,noise suppression plans require a chute or side baffles. • All vehicular movements to and from the site must only be made during the scheduled normal working hours.This includes off-site noise that is associated with a specific project such as staging of concrete trucks. Shields,shrouds,or intake and exhaust mufflers. 2. Noise-deadening material to line hoppers,conveyor transfer points,storage bins, or chutes. 3. Noise barriers using materials consistent with the Temporary Noise Barrier Materials Section. 4. Noise curtains 5. Plywood with concrete blankets at the height of the equipment and that it surrounds the activity such that it directs noise up more than out from the property. 6. Portable three sided enclosures made out of plywood to move with the activity such as jack hammering. 7. Internal combustion engines are to be fitted with a suitable muffler in good repair. Specific Equipment: Generators: The local power grid shall be used wherever feasible to limit generator noise.No generators larger than 25 KVA shall be used and,where a generator is necessary,it shall have maximum noise muffling capability. Backup Alarms:All equipment with backup alarms operated by the Contractor,vendors, suppliers,and subcontractors on the construction site shall be equipped with either audible self-adjusting ambient-sensitive backup alarms or manually- adjustable alarms.The ambient-sensitive alarms shall automatically adjust to a maximum of 5 dBA over the surrounding background noise levels.The manually-adjustable alarms shall be set at the lowest setting required to be audible above the surrounding noise.Installation and use of the alarms shall be consistent with the performance requirements of the current revisions of Society of Automotive Engineering(SAE)J994,J1446,and OSHA regulations. Compressors: The unit with the lowest noise rating which meets the requirements of the job should be used where work is conducted in the City of Aspen,installed with mufflers and/or enclosed in a noise barrier. Concrete crushers or pavement saws: Pre-augur pile holes to reduce the duration of impact or vibratory pile driving and tie to local power grid to reduce the use of generators and shall be enclosed with shields or acoustical barrier enclosures. Pneumatic hand power tools: All pneumatic tools operated in the City of Aspen must be fitted with an effective silencer on their air exhaust port. Temporary Noise Barrier Materials: Temporary barriers shall be constructed of 3/4-inch Medium Density Overlay(MDO)plywood sheeting,or other material of equivalent utility and appearance having a surface weight of 2 pounds per square foot or greater.The temporary barriers shall be lined on one side with glass fiber,mineral wool,or other similar noise curtain type noise-absorbing material at least 2-inches. The materials used for temporary barriers shall be sufficient to last through the duration of the construction project,and shall be maintained in good repair.Prefabricated acoustic barriers are available from various vendors.An equivalent barrier design can be submitted in lieu of the plywood barrier described above. Impact Equipment: Impact noise is noise produced from impact or devices with discernible separation in sound pressure maxima.Examples for impact equipment include,but are not limited to;blasting,chisel drops,mounted impact hammers(hoe ram),and impact pile drivers. Impact equipment is the loudest and most intrusive to the neighboring property. The City of Aspen requires that this type activity have the strictest mitigation requirements and requires a customized noise suppression plan specific to the site.General contractors must contact the City of Aspen Environmental Health Department for an application at 970-920-5039. Noise Control • Replace worn,loose,or unbalanced machine parts that cause vibration. • Keep machine parts well lubricated to reduce friction. • Acoustical enclosures and barriers around generators • Sound absorbing material and vibration isolation systems on hand tools • Quiet work practices-use rubber mallets to erect and dismantle formwork.
