The insurance document you uploaded is an affidavit of insurance for temporary encroachments.Permanent encroachments must provide the actual proof of insurance.I have attached an example for your reference. Furthermore I see Permit 0001-2021-ENPM was submitted for 612 W Francis by Joel Hocknell.On March 18th,2021,it was determined"ROW permit needed to install snow melted walkway.After walkway is in,we will need a survey of what is encroaching and then we can move forward with a permanent encroachment license."
