Materials were sampled to confirm or deny asbestos content. Materials were sampled using methods described in Secon IV.C. of the CDPHE Regulaon 8. Date of Inspecon: 12/22/2023 Date of Report: 12/22/2023 Scope of Inspecon: This report was generated to idenfy ACMs (asbestos-containing materials) present within the bathroom wall to assist in a remodeling project. The Homeowner plans on removing a dividing wall in the bathroom. The owner also communicated that a poron of the bedroom ceiling might be removed. Further tesng would need to be performed if a poron of the ceiling is going to be disturbed. The materials tested were in a non-invasive manner of all easily accessible materials. If materials beyond the scope of this inspecon are discovered, those materials should be considered ACMs per Colorado’s regulaons and be tested by a cerfied building inspector to prove otherwise. 02/20/2024 Field Procedures & Analysis Methods: Guidelines used for the inspecon were established by the Environmental Protecon Agency (EPA) in the Guidance for Controlling Asbestos Containing Materials in Buildings, Office of Pescides and Toxic Substances, DOC #560/5-85-024 and 40 CFR Part 763, Asbestos Hazard Emergency Response Act (AHERA). Field informaon was organized as per the AHERA concept of Homogeneous Area. A Homogeneous Area is defined as a suspect material of similar age, appearance, funcon, and texture. Each material was grouped together as a specific Homogeneous Area, sampled, and then assessed for condion. Bulk samples were collected in a stascally random manner, described in the EPA “Pink Book” (560-5-85-30A). Bulk samples of suspect ACM were analyzed by Polarized Light Microscopy (PLM). Homogenous Area Types & Sampling Requirements: Surfacing Material: Less than 1,000 square feet – minimum 3 samples to be collected Between 1, 000 – 5,000 square feet – minimum of 5 samples to be collected Greater than 5,000 square feet – minimum of 7 samples Miscellaneous Material: A minimum of two samples of each homogenous area of miscellaneous material must be collected Thermal System Insulaon (TSI): A minimum of 3 bulk samples are to be collected per homogenous area of TSI, at least one sample from each homogenous area of patched TSI that is less than 6 square or linear feet, and in a manner sufficient to determine whether the material is ACM, collect bulk samples from mechanical system fings such as tees, elbow, or valves. Collected Assessment Categories: 1. Damaged or significantly damaged Thermal System Insulaon (TSI) ACM 2. Damaged friable surfacing ACM 3. Significantly damaged friable Surfacing ACM 4. Damaged or significantly damaged friable Miscellaneous ACM 5. ACM with potenal for damage 6. ACM with potenal for significant damage 7. Friable ACM with low potenal for damage Other- ACM not classified in the above Assessment Categories N/A – Non-Applicable 02/20/2024 Homogenous Areas & Collected Samples Locaons: Materials were assigned to homogenous systems based on their associated building system, material type, texture, friability, and proximity to one another. Each material was felt and gauged on whether materials could be crushed by hand pressure into fibrous or dust-like components. The condion of each homogenous area was recorded based on the damage present at the me of inspecon. Homogenous Area Descripon Of Area ACM Category Number of Samples Approximate Square Footage ACM Assessment Category Percentage & Asbestos Type A1-3 Off-White Orange peel Texture in the bathroom areas Surfacing 3 ~48 sq N/A N/A M1-2 Drywall system behind the walls W/ off- white orange peel texture in the bathroom areas Miscellaneous 2 ~48 sq N/A N/A Conclusions: Laboratory analysis confirms sample set A1-3 is non-detect for asbestos. The sample set at the me of inspecon was in good condion but did have a 1x1 sq patch. 3 samples were taken analyzed the orange-peal texture on the common wall between the 2 secons of the bathroom. Renovaon of this building material can be accomplished without an abatement. The applicable materials have been highlighted as bronze in the sample diagram. Laboratory analysis confirms sample set M1-2 is non-detect for asbestos. The sample set at the me of inspecon was in good condion but did have a 1x1 sq patch. 2 samples were taken to analyze the drywall system W/ orange peel on the common wall between the 2 secons of the bathroom. Renovaon of this building material can be accomplished without an abatement. The applicable materials have been highlighted as bronze in the sample diagram. This is by no means a comprehensive building inspecon. If more materials are to be affected and removed that are suspect ACM or ACBM that were not sampled - work must cease, and further sampling must occur. Please let us know if you have any quesons or concerns. 02/20/2024 1 2023-800MILLST#305A 12/22/2023 Page: 1 General Conditions Bathroom Pt 2 A2M1 Bathroom Pt 1 A1 A3 M2 Cabinet1 (B1) Cabinet2 (B1) Bathroom Pt 2 A2M1 Bathroom Pt 1 A1 A3 M2 Cabinet1 (B1) Cabinet2 (B1) General Conditions 5' 5 " 8' 2" 8' 10" 4' 9 " 3' 3 " 6' 1" 2' 1 1 " 8' 7 " 5' 5"2' 9" 2' 02/20/2024 2 2023-800MILLST#305A 12/22/2023 Page: 2 View - With Dimensions BATHROOM_ANGEL_1 02/20/2024 3 2023-800MILLST#305A 12/22/2023 Page: 3 View - With Dimensions BATHROOM_ANGEL_2 02/20/2024 4 2023-800MILLST#305A 12/22/2023 Page: 4 View - With Dimensions BATHROOM_ANGEL_3 02/20/2024 5 2023-800MILLST#305A 12/22/2023 Page: 5 View - With Dimensions BATHROOM_ANGEL_4 02/20/2024 December 22, 2023 Sincerely, Jeanne Spencer President Subcontractor Number: Laboratory Report:RES 587384-1 Project #/P.O. #:None Given Project Description:800 Mill St Unit 305A Gavin Mcgonigle Helpers Disaster Restoration, LLC 11 Duroux Lane Basalt CO 81621 Dear Gavin, Eurofins Reservoirs is an analytical laboratory accredited for the analysis of Industrial Hygiene and Environmental matrices by the National Voluntary Laboratory Accreditation Program (NVLAP), Lab Code 101896-0 for Transmission Electron Microscopy (TEM) and Polarized Light Microscopy (PLM) analysis and the American Industrial Hygiene Association (AIHA LAP, LLC), Lab ID 101533 for Phase Contrast Microscopy (PCM) analysis.
