1.0 GENERAL
I affirm that the project contractor has completed the City of Aspen online CMP
certification class.A copy of the CMP certificate is provided with this submission.See `EXHIBIT A.'

3.0 PROJECT DOCUMENTATION
Permits Related to this Project:#24-1034; no encroachment permit will be needed.
Indicate Project Public Notification: Residents within 300 feet of project will be mailed
project notifications describing the construction projects scope of work and their
relative tentative schedule.The letter will also inform the recipients of the project
representative's contact information; no less than 10 days prior to the start of the
project with project updates to occur at least monthly.
Additionally, Project Sign Will Be Posted at Site Per Requirements Set Forth by City of
Aspen. See `EXHIBIT C'
4.0 PROJECT IMPLEMENTATION
Dates of Project Construction: Foundation excavation and waterproofing was completed before
Aspen Home Inspections became the general contractor.We would like to complete backfill,final
grading and revegetation by June 2024.
Hours of Construction: Construction will be limited to 7:30am-5:30 pm Monday
through Friday, and 9:00am-5:00 pm on Saturday. No construction will be completed
on Sundays or holidays as identified by the CMP Manual.
Will construction fencing be provided?Yes. See `EXHIBIT B.'
Are there trees within the work area: Yes. See `EXHIBIT B.'

5.0 SITE AND RIGHT OF WAY MANAGEMENT
A Site plan is attached which shows all construction staging areas.
Due to our limited construction schedule and space,we will not be utilizing a
dedicated construction trailer at this site. See `EXHIBIT B.'
Will the project use any portion of the Public Right of Way for construction purposes?
The project shall submit a temporary encroachment permit for the use of the ROW.—
No
Will the project maintain continuous emergency vehicle access, on and around site,
including by not limited to police,fire, and ambulance services?-Yes
Temporary Encroachment Details—No anticipated plan for temporary encroachment at this property.
Does the project require leasing public parking spaces for construction purposes? -
No
Does the project propose earth retention? -No
The City of Aspen requires recycling of materials, both conventional and construction
related. Does the project have a recycling plan in place on site? - Yes
6.0 TRAFFIC CONTROL
Does the project require a traffic control plan?- No
Does the project require any special circumstances for deliveries? - No
7.0 PEDESTRIAN PROTECTION
Does the project maintain pedestrian walkways including all ADA requirements?-Yes

Is the project located in the Smuggler Mountain Superfund Site? No
9.0 FUGITIVE DUST CONTROL
Yes.A water hose will be used to keep the dust under control during excavation.

All vehicles and equipment used on site will be properly maintained such that the engines will
function within manufacture's standards or parameters.Yes

A noise suppression plan is not applicable to this project.
