The Client, John Monkarsh, is planning a renovation-remodel-demolition of the entire
interior space of the above addressed townhome. Jim Baker of HealthSafe Inspections

Inc. (hereinafter, HealthSafe)performed an inspection for the property in October of
2016 on behalf of the Client through the contractor, William H Baker Construction, Inc.
Due to the inspection, sampling and report being several years old, it was required that
Jim Baker return to the property and inspect those materials that were tested and
determine if there have been any changes since that original inspection on October 3,
2016.
Jim Baker returned to the property on Monday, June 28th, 2021.An inspection of the
homogeneous materials throughout the townhome were re-inspected. There were NO
changes to those materials since 2016, and no additional testing was necessary.All
interior homogeneous materials are accounted for in the October 2016 Asbestos Report
dated 10/25/2016.
