General Remarks:
As requested, a site visit was made to observe drilled and epoxied anchor installation. The four remaining holes were drilled
into an existing concrete slab after demolition of the existing concrete ramp. We observed contractors clean the holes using a
wire brush and compressed air prior to epoxy application. The contractor was notified of our observations. Our office will be
contacted when additional services are required.
