Please make the following changes to the Block 9 Plat:
1. Include a signed title certificate.
2. Change the name of Serviceberry Drive to Five Trees Lane (change.
approve per City Council Resolution No. 76, Series of2001.
3. The City Engineer's signature block needs to be changed to read
"Community Development Engineer".
4. Notary Stamps are required on the Mylars rather than on stickers
attached to the Mylars.
5. Obtain all signatures with the exception of the City Community
Development Director, Community Development Engineer, and
Pitkin County Clerk and Recorder prior to submitting mylars to
Community Development Department.

To Whom It May Concern:
We propose to amend the location of the building envelope on Lot 9, Block G, in the
Moore FamJy PUD. This is necessary for the following reasons:
1. The access ease~ent for Lot 8, which was amended February 13, 2001 (see B.56,
P.62), cut through the original Lot 9 buJding envelope.
2. The access and sewer easement that extended from Serviceberry Drive is now only a
sewer easement and no longer provides access to Lot 8 and 9.
Our proposal is to relocate the Lot 9 building envelope away from tI,e Lot 8 access
easement while maintaining the square footage of the original easement.
By vote of the prop~rty owners, approval was granted by the Five Trees Design Review
Board. This letter is attached.
We reviewed this with Sarah Oates, City Zoning Officer, who informed us that this
proposal would be considered an "Insubstantial Amendment" on Ap:rJ 10, 2002.
