Although the applicant has submitted line locate reports from three of the six utilities, what is needed is either letters from all six utilities or sign -off certificates on the plat amendment which grant permission to vacate the easement. 2. The plat amendment must contain a note which states what action was effected by the amendment and at what book and page the original plat and any subsequent amendments are filed. 3.
